At Care Concierge, we understand the critical role of comprehensive stroke management in your recovery journey. The first three months after a stroke are especially crucial. This is when your brain’s neuroplasticity—the ability to form new neural connections—is at its peak. It’s a prime time for early and intensive rehabilitation to make a real difference.
But recovery doesn’t end after three months. Significant progress can continue for six months or more with ongoing support and rehabilitation. Our goal is to assist stroke survivors in achieving the best possible outcome by considering their stroke symptoms and recovery needs in developing long-term strategies for stroke rehabilitation.
We take a holistic approach to care, assigning you a dedicated Care Manager who will craft a personalised, long-term recovery plan. This plan goes beyond physical rehab and nursing care for stroke patients. Your mental and emotional well-being, nutrition, and lifestyle changes are taken into account too. Whether it involves managing daily routines or improving mobility through exercises for stroke patients, we’re committed to improving your quality of life and recovery.
Whether your goal is to strive for full recovery or to manage your current abilities and quality of life, we’re here to support you every step of the way. With Care Concierge, you’re not just receiving care—you’re partnering with a team dedicated to helping you reach your highest potential in recovery.

Having a good bed
Installing side rails or renting a medical bed with adjustable height and positioning to support proper posture and prevent falls.
Ripple Mattress
For bedridden patients, a ripple mattress can help prevent bedsores by improving circulation on the back, ensuring oxygen flow and reducing the risk of skin issues.
Wheelchair and Mobility Aids
Depending on the individual’s mobility level, a wheelchair (manual or electric) or a walking apparatus may be necessary to facilitate movement around the home.
Door and Safety Modifications
Doors should open outwards to prevent being blocked by a fall. Protect against injuries by covering sharp corners and removing trip hazards. If the home has steps, consider installing ramps or railings to aid mobility.
Bathroom Modifications
Ensure the bathroom is easily accessible. This may include installing grab rails and adding a commode chair for those who need assistance in using a traditional toilet, and making sure the flooring is anti-slip. For the shower area, consider a shower seat or chair, and avoid using bathtubs.
