The published examples in this guide start from RM2,500 a month for a shared room in a private care home and reach RM5,000 and above for a private room. Some hotel-style assisted living options publish starting fees above RM10,000. These are selected examples, not a nationwide minimum, maximum or average. The care required, room and extras determine your parent's quote.
If you are pricing care for a parent, you have probably noticed that every home quotes differently and few say what the fee leaves out. This guide puts published prices side by side, explains what moves the number, and shows how families use tax relief and EPF savings to pay for it.
Nursing home prices in Malaysia by type of home
Care Concierge publishes this guide. The table groups selected providers with public prices by type of home. It is a price illustration, not an independent ranking or a complete market survey. Prices are as published on each provider's own website in October 2026 and may change.

Two notes on the table. Sunway Sanctuary's own site gives two starting prices for assisted living: RM10,800+ a month in its FAQ and RM13,300+ on its assisted living page. Ask which applies to the suite you are shown. And JKM describes Rumah Seri Kenangan as a last resort for seniors who are neglected and need protection. Applications go through a district welfare office and depend on vacancies, so most families with working children budget for a private home instead.
What the monthly fee usually covers
At a residence, the monthly fee normally covers the room, meals, help with daily activities such as bathing and dressing, housekeeping and laundry. Here is what our own brochures list:
- The Mansion: professional caregivers for daily activities and companionship, a senior nurse on site, an occupational therapist, two meals a day cooked by a professional cook plus breakfast, tea and snacks, housekeeping and laundry, and the Care Concierge app for health and vital records. - Tanjung Point Residences: 24/7 professional caregivers supervised by nurses and other specialists, social and cognitive activities, all meals, Astro and Wi-Fi, and weekly housekeeping and laundry.
- Angsana Home: 24/7 trained caregivers, registered nurse support, daily programmes led by a physiotherapist and caregivers, all meals, Wi-Fi, and daily housekeeping and laundry.
What often costs extra
Most homes charge some items on top of the monthly fee. Genesis Life Care, for example, states that consumables such as diapers and feeding formula, specialist medication, doctor or specialist visits and physiotherapy are itemised separately.
Ask every home for an itemised quote. The monthly fee is only comparable once you know which of these extras your parent will need each month.
What pushes the price up or down
Level of care
The more hands-on help your parent needs, the higher the fee. Genesis Life Care's published packages show the pattern clearly: Essential Care from RM2,500, Assisted Living from RM3,000, Nursing or High-Dependency from RM3,800, Complex and Palliative from RM4,500, and a private room at any care level from RM5,000 a month. Bedridden care and tube feeding sit in the higher bands.
Room type
Sharing a room lowers the fee. Genesis Life Care's shared rooms start from RM2,500 a month, while a private room at any care level starts from RM5,000. Ask each home for the difference between room types at the care level your parent needs.
Setting and location
The type of home matters more than the state. Within one city, a converted bungalow and a sea-view or hotel-style residence can sit in very different price bands. The gap between private care homes and premium assisted living in the table above shows how wide that can be.
Who is on duty
Homes describe their staffing differently: caregivers only, caregivers supported by nurses, or a nurse on site. Ask who is on duty at night, and whether a nurse is in the building or on call. This affects both the fee and whether the home suits your parent's condition.

Nursing home or home care: which costs less?
A residence fee usually bundles accommodation, meals and daily support. A quote for care at home covers agreed care hours; the household still pays for food, utilities, equipment and any hours not covered by family or the care team.
Compare two written quotes for the same needs and schedule. A few hours of help at home can cost less than moving into a residence. A parent who needs support throughout the day and night needs a different comparison: check active care hours, rest periods and overnight cover before treating a live-in arrangement as continuous care.

How families pay for nursing home fees
Income tax relief for parents' care
For year of assessment 2025, the latest year on LHDN's relief list, eligible taxpayers may deduct up to RM8,000 for medical treatment, dental treatment, special needs and carer expenses for parents and grandparents. Complete medical examinations, including vaccination, count within a RM1,000 limit. LHDN's public ruling includes care services provided by a care home, and care at home or in any care centre.
The conditions matter:
- A doctor registered with the Malaysian Medical Council must certify that your parent's condition needs treatment or a carer.
- Your parent must be resident in Malaysia, and the care must be provided in Malaysia.
- The deduction is on expenses you paid yourself, so keep receipts in the name of the child who pays.
Tax relief reduces taxable income; it is not an RM8,000 cash payment, and not every part of a residence bill automatically qualifies. Confirm the eligible care expenses and documents with LHDN or a tax adviser. If siblings share the monthly fee, check with LHDN or a tax adviser how each of you should claim before you file.
EPF Akaun Fleksibel (Account 3)
If you are below 55, you can withdraw from your EPF Akaun Fleksibel at any time, with a minimum of RM50 per withdrawal, through the KWSP i-Akaun app, the i-Akaun portal or an EPF office. Withdrawal reduces your retirement savings. Check the available balance and your longer-term budget before using it for care fees. EPF reminds members to use it with care so that retirement savings stay adequate.
Insurance
Before you count on insurance, ask your parent's insurer in writing whether any part of a residence stay is covered, and under which benefit.
Questions to ask before you sign
Deposits, admission fees and notice terms differ from home to home and are rarely published. Ask each home for these answers in writing:
- Deposit: how much, and is it refundable when your parent leaves?
- Admission or registration fee: is there one, and what does it cover?
- What the fee includes: meals, laundry, activities, the care team.
- What costs extra: diapers, medication, doctor visits, physiotherapy, hospital escorts, special diets.
- Night cover: who is on duty overnight, and is a nurse on site or on call?
- Changing needs: how the fee changes if your parent needs more care later.
- Notice period: how much notice to leave, and what happens to fees during a hospital stay.
- Short stay: can your parent try a short stay before committing?
- Receipts: will receipts itemise care services clearly enough for a tax relief claim?

Can I claim tax relief for nursing home fees?
Eligible care expenses may qualify, subject to LHDN conditions. For YA 2025, the restricted deduction for parents' and grandparents' medical treatment, special needs and carer expenses is up to RM8,000. It reduces taxable income and is not a cash payment. Medical certification and receipts are required; ask LHDN or a tax adviser which parts of your bill qualify.
Is a nursing home cheaper than a live-in caregiver?
Compare written quotes covering the same needs and hours. A residence fee usually bundles the room, meals and daily support. Care at home leaves household costs and any uncovered hours with the family. Ask about active care hours, rest periods and night cover; a live-in caregiver is not automatically continuous hands-on care.
How much is a nursing home in Penang?
Fees depend on the type of home, the room and the care your parent needs, so ask each home for a written quote.
