Overview
A protocol to evaluate skin condition and identify risks of pressure ulcers, wounds, and other skin-related issues.
About the Skin Assessment
This protocol is essential for evaluating the condition of an elderly care recipient's skin, particularly to identify risks of pressure ulcers, wounds, and other skin-related issues.
For high-risk individuals, perform skin assessments daily. For others, a weekly check may suffice.
Record any changes in skin condition over time, noting specific locations and descriptions of concerns.
If redness, sores, or any open wounds are found, take immediate steps to relieve pressure, apply wound care, and notify a healthcare provider if necessary.
A higher score indicates more significant skin concerns. 10-14 suggests moderate concerns requiring a regular skin care regimen and possible medical consultation. 15-20 indicates high risk, requiring immediate intervention and referral to a specialist.

4. 4. Elasticity/Turgor
Pinch a small section of skin (usually on the forearm) to test elasticity
"Does the skin quickly return to its normal position after pinching?"

Ask about any numbness or tingling sensations
"Do you feel any numbness or tingling in certain areas?"
6. 6. Pressure Ulcer Risk
Inspect common pressure points (heels, sacrum, elbows) for redness, bruising, or sores
"Are there any red, swollen, or sore spots on pressure points?"
7. 7. Edema (Swelling)
Press gently on the skin over bony areas to check for pitting
"Does pressing on the skin leave an indentation?"
